Nursing Home Administrator
- Manages the overall direction, coordination, and evaluation of the Operations & Human Resources Departments.  Carries out supervisory responsibilities in accordance with the company policies and applicable laws.  Responsibilities include interviewing, hiring, planning, assigning, directing work, rewarding, disciplining employees, performance reviews and addressing complaints and resolving problems.  Ensuring day-to-day operations are in accordance with State and Federal guidelines & regulations that govern the facility; is required to ensure the highest degree of quality care is maintained at all times.

Qualifications include a four-year degree, preferably in Business Administration, Public Health, Public Health Administration, Nursing, Social Work, or a similar or related field as well as current licensure as a Nursing Home Administrator, both State and Federal, or the ability to obtain these licenses within 12 months.

MDS Coordinator (Registered Nurse - RN) -
Works under direction of Director of Nursing to coordinate revenue stream and accurately report information to CMS.  Is responsible for management of RAI process, schedule PPS, and OBRA MDS; issues Medicare Denial forms as required by regulation, train staff in MDS implementation, get prior authorization from insurance companies for skilled benefits for residents.  MDS Coordinator supports or participates in the following programs:  Resident Care Conferences, Admission/Discharge Planning, Quality Assurance, Utilization Committee, Fire and Safety, Infection Control, Employee Health Program, Medication Review, and will formally participate in evaluation of each of the above listed programs as it relates to resident care and overall departmental activities.  Desired candidate would also have experience with wound care and implementing a restorative program.  Maintains an organized system for completing tasks.  Participates in facility operations as a team leader.  Some weekends required (on-call and on-site).  These job duties are not intended to be all inclusive.  Requirements:  Current MN license as RN, experience in long-term care setting favored, reference given to experience in MDS/RAI or clinical reimbursement.

     Position is full-time (40 hours per week) every 3rd weekend and holiday rotation requirement.

Infection Preventionist (Registered Nurse - RN) -
Works under the direction of the Director of Nursing.  The Infection Preventionist is responsible for the implementation and review of the facility's infection prevention program as mandated by federal and state regulations.  The IP collects and analyzes infection and treatment data and completes audits to ensure compliance with current policies and procedures.  Additionally, the IP will perform the reporting of infection and quality data for various facility state and federal reporting requirements.

We are searching for a highly energetic individual who demonstrates a desire to plan and conduct employee training and exercises and has a working knowledge of nursing practice and theory, specifically in regard to infection control.  The IP should be adaptable and flexible in order to meet a wide range of service needs within our facility.

Participates in facility operations as a team leader.  These job duties are not intended to be all inclusive.  Requirements:  Current MN license as RN, experience in long-term care setting favored.  Benefits include health insurance, life insurance, Colonial Life, Flexible Spending, PTO, and 401(k) if eligible.

     Position is full-time (32 hours per week) every 3rd weekend and holiday rotation requirement.



 Registered Nurse (RN)
- The role of the RN is to ensure that the health and safety of all residents are met through weekly on site visits with the residents and staff.  The RN will ensure that staff s trained in medical procedures and policies which are current and up to date.  The RN will also ensure that the facility is in compliance with current clinical regulations and standards.  Must have a desire to work with the seniors.  Must be compassionate, patient, reliable, self-motivated and have good communication skills.  Ability to communicate well with residents and families explaining medical needs/changes related to resident's care as well as advocate for the proper services to be provided.  Must be able to relate professionally with residents, staff and family members, display a positive work attitude and be able to work as a team with other staff members.  Will work every other weekend and holiday rotation.  Position requires current RN licensing in the state of MN.  Benefits include Paid Time Off (PTO) and 401(k), if eligible.

    Casual - Orientation will be provided for all 3 shifts. 


Licensed Practical Nurse (LPN) Union Position
- Under the direction of an RN, the LPN will provide supervision of all NA staff along with monitoring and providing appropriate care to the residents, and have good communication skills.  Duties to include but not limited to:  medications, injections, dressing changes, contacting families and physicians, staffing, and any delegated duties within the scope of practice.  Will work weekends as needed (not more than every other weekend) and holiday rotation.  Position requires current MN LPN licensing.  Benefits include Paid Time Off (PTO) and 401(k), if eligible.

      Full-time evenings (2-10:30pm  8 shifts/pay period 

       

Certified Nursing Assistant Union Position -  The nursing assistant is responsible for assisting all licensed nurses in the delivery of resident care.  Under the direction of the nurse, the nursing assistant is responsible for all personal care, with the emphasis on the activities of daily living of a group of assigned residents and records, and reporting unusual findings to the nurse.  Position requires nursing assistant certification or ability to complete class and pass exam within four months of hire (company will provide training or payment for training to become certified).  Every other weekend and holiday rotation required.  Benefits include Paid Time Off (PTO) and 401(k), if eligible.
                   
                     
       Part-time Day Shift (6 am - 2:30 pm  6 shifts/pay period)
        
       Full-time Evening Shift (2-10:30 pm 8 shifts/pay period)

       Part-time Overnight Shift (10:00 pm - 6:30 am)


      

Trained Medication Aide (TMA) Union Position - The Trained Medication Aide (TMA) is responsible for the delivery of resident care.  Under the direction of an RN or LPN, the TMA is responsible for distributing medications to the correct residents at the assigned times, assisting residents with medication and treatment-related needs, and other tasks within their scope of practice.  The position requires a current nursing assistant certification and passing a TMA course that is accepted in Minnesota with good standing.  Every other weekend and holiday rotation required.  Benefits include Paid Time Off (PTO) and 401(k), if eligible.

       Part-time Evening Shift (2-8 pm)
     

Dietary Aide Union Position
- Under the direction of the cook and Dietary Manager, the Dietary Aide will help coordinate and complete preparation and serving of snacks and meals, to ensure nutritional needs of residents are met, as well as assisted in maintaining kitchen work areas and keeping equipment, utensils, and food plates clean and orderly.  Will work every other weekend and holiday rotation.  Benefits include Paid Time Off (PTO) and 401(k), if eligible.

Cook - Prepare nutritious meals for residents, serve and clean up resident meals, prepare baked goods for snacks, prepare for following day.  Will work every other weekend and holiday rotation.  Benefits include health insurance, life insurance, Colonial Life, Flexible Spending, PTO, and 401(k), if eligible.

Housekeeping/Laundry
- Clean resident rooms, common areas, assists with laundry, and other tasks as needed.  Day shift will work every other Saturday and holiday rotation.  Benefits include Colonial Life, Paid Time Off (PTO), and 401(k), if eligible.

- Maintenance worker would mow lawn, clear leaves, wax floors, repair wheelchairs, and assist with other maintenance activities.
